Required Swimming/Diving Participation Forms
There are 3 forms that need to be completed before any participation in swimming and diving events, including practices. These forms can be downloaded by clicking the links associated with each. If you have not already done so, please fill them out and turn ALL 3 into Coach John at the preseason "Kick off Meeting" on October 4th.

Physical Form

MUST be completed every year and will carry over from any fall sport or band. If carried over, Sport/Activity must be told to Coach John IN WRITING when submitting forms

Code of Conduct

MUST be completed every year and will carry over from any fall sport or band. If carried over, Sport/Activity must be told to Coach John IN WRITING when submitting forms

Emergency Medical Form

MUST be filled out for every sport, regardless of previous participation in a sport or activity
PLEASE NOTE:

Forms will NOT be accepted 1-2 at a time...all 3 forms MUST be turned into Coach John at the same time!!

If you do not hand in your forms to Coach John at the "Kick off Meeting" you may drop them off at the Washington Township Recreation Center pool on the following dates from 3:30-5:30PM:

October 8th, 10th, 15th, 17th, 22nd, 24th

Without all 3 forms, registration will NOT BE PROCESSED and your swimmer/diver will NOT BE CLEARED to compete or practice for the Elks.
Away Meet Transportation:
For away meets only, all swimmers must use CHS school transportation to-and-from meets. If for any reason other transportation is necessary, the following is the proper procedure to follow:

The form must be signed by the athlete, parent, and coach

Turn the form into the Athletic Director in the Athletic Office for signature

After the Athletic Director signs it, return the form to Coach John. The athlete will not be permitted to travel to or from the meet without the team without turning in this form
